Our friendly, caring staff provides both long and short-term skilled nursing care for people recovering from surgery, wounds, injuries or other medical conditions. Emphasis is placed on quality of life and spiritual and emotional support as well as medical care.

Long-Term Skilled Nursing Care residents receive the unique day-to-day services they require so that they can live out their years with dignity and the highest possible quality of life available to them.

Admission to GreenField Health and Rehabilitation Center is the beginning of a new life for many of our residents. We take great care to make an attractive, homelike environment, and we encourage our residents to socialize.


We welcome their loved ones to spend quality time with them. Anniversaries, birthdays and other special celebrations are encouraged.

Short-Term Skilled Nursing and Sub-Acute Care residents receive specialized care to help them achieve as complete a recovery as possible. Most of these residents are recovering from surgery, wounds, injuries or other medical conditions.

Working closely with their physicians and therapists, specific programs are designed so that each individual has the opportunity to maximize his or her potential.

Our social services department helps these individuals with discharge planning and arranging for community services and any special equipment they may require.
