Rehabilitation Services
Subacute rehabilitation is an invaluable inpatient program offered to patients who no longer require the acute care provided in a hospital setting. Following hospitalization, it is the next step in the continuum of care and is coordinated by an interdisciplinary team of professionals who specialize in various areas of rehabilitation. At the Niagara Lutheran Home & Rehabilitation Center, once a patient has been evaluated, our team meets weekly to review progress and ensure each patient is on target to meet his or her individualized goals and returns home in the shortest period of time.
Who Benefits from Subacute Rehabilitation Services?
People who experience the following:
- All orthopedic surgeries including joint replacement
- Back pain and spinal injuries
- Amputations
- Cardiac conditions
- Neuromuscular conditions
- Cerebral vascular accidents (strokes)
- Motor vehicle accidents
- Arthritis
- Generalized deconditioning due to acute or chronic illness
Criteria for Admission
Inpatient subacute rehabilitation services are covered by most insurance plans, provided that the following criteria are met:
- Patients must have the ability and be willing to actively participate in a rehabilitation program
- Patients must be cleared medically to participate
- Patients must be able to show reasonable and continued progress for their diagnosis
- Other criteria and/or limitations may apply depending on the individual's insurance coverage
The Niagara Lutheran Home & Rehabilitation Center accepts and encourages pre-registration for patients having elective surgery. This gives patients an opportunity to tour the facility, have questions answered prior to surgery, and expedites the admission process.
