GreenField Health & Rehabilitation Center

Greenfield Health & Rehabilitation Center is part of The GreenFields Continuing Care Community. This 162-bed facility, which opened in 1998, complements the residential apartment homes and assisted living services offered by GreenField Manor and GreenField Court on our beautiful 52-acre campus in Lancaster, New York.

At the GreenField Health & Rehabilitation Center, every resident benefits from an individualized plan of care developed and carried out by an interdisciplinary team that meets regularly to discuss progress and update the plan of care as necessary. The team is comprised of the resident and his or her family members, as well as our medical director; physical, occupational and speech therapists; and nursing, activities, nutrition and social services staff. If it is determined that a resident is able to go home or to a different level of care, our social services staff will help with discharge planning and arranging for community services and special equipment that might be necessary.

The GreenField Rehabilitation Department has earned a reputation as the premier provider of rehabilitation services in Western New York. Rehabilitation and subacute care patients are cared for on their own special wing so they receive the privacy and specialized nursing care they need. Our staff physician meets regularly with patients to ensure that they are progressing to the best of their ability. Many patients find that the familiarity of staff and procedures when they continue with outpatient therapy at The GreenFields helps them to achieve the best possible results.
