The Buffalo City Campus, conveniently located in the heart of Buffalo near Canisius College and Sisters Hospital, is comprised of two facilities − the Lutheran Church Home assisted living residence, and the Niagara Lutheran Home & Rehabilitation Center, which provides rehabilitation services, and skilled nursing, subacute, respite, and Alzheimer's/dementia care.

Residents of both facilities receive priority access to various levels of care on the campus and within the Niagara Lutheran Health System.
